Advance Health Care Directives don’t directly affect your
finances, if you don’t have one, become incapacitated, and
can’t express your wishes, an expensive court proceeding may
be required. And your family could be torn apart
unnecessarily.
By
completing My Final Decisions™ Advance Health Care
Directive, you will be able to clearly describe the type and
extent of medical treatment that you prefer - and don’t prefer
- so those you trust can carry out your instructions if you
are unable to express them.
Unlike
Living Wills that are generally invoked only in end-of-life
situations, My Final Decisions™ Advance Health Care
Directives allow you to set out your instructions for any
time you may be unable to express your wishes. For example,
you might be unconscious, under anesthesia, have dementia,
or be under heavy sedation or pain medication and be unable
to state your preferences. My Final Decisions™ allows you
to plan for these events.

Put in writing your
directions about what medical treatment you want
not only at end of life -- such as
resuscitation, ventilators, and artificial
feeding – but also when you may be
unconscious or otherwise unable to state your
wishes. You will be able to state who can – and
who can’t talk to your doctors and medical
providers through comprehensive medical

Appoint a trusted agent to make your medical
decisions on your behalf, have a procedure to
replace agents who do not follow instructions,
and avoid disputes over who will visit you
should you be incapacitated. |

State your wishes
about organ donation for transplantation; give
directions about how and where you want to be
buried or how you want your remains disposed of
and who will be in charge; and avoid disputes
about your health care if you can’t express your
wishes. |

spouse, family, partner, or friends guidance on
how you wish to be treated; avoid disputes; and
assure you will receive the medical treatment
and quality of life you want and deserve. |
